You must be able to present yourself (your character) very authentically without a trace of overacting. You must engage a feeling inside of you that feels very unsatisfied with how the therapeutic relationship is going right now in this moment of a psychotherapy session. Actors with a history of experiencing bad therapy are a plus here. Each vignette is ultra-brief and you must be convincingly angry, depressed/hopeless, shutdown, or highly anxious. You are presenting a very difficult/challenging moment for a potential therapist to respond to.
